Disclaimer & Communication Policy

PARTICIPATION POLICY

I, the undersigned parent or guardian of the enrolled named child(ren), acknowledge, agree and understand that my child is in good health and suitable physical condition to participate in strenuous physical activities, including soccer. There are certain risks and hazards involved when participating in soccer and physical activities that may result in injury or illness to either my child or other participants including, but not limited to those hazards associated with, playing conditions, equipment, other individuals and exposure to bacteria/viruses. If my child is unwell or showing symptoms of Covid-19 I must notify UK International Soccer Camps head office and I understand that my child will not be allowed to attend the program. Should my child be recovering from Covid-19 symptoms a physician must provide written clearance, which I must submit in writing to UK International Soccer Camps for approval in order to attend the program. Permission is granted for my child to receive emergency medical treatment, if needed, and I certify that there are no limits to my child(ren)’s participation except as stated in writing and included with this registration. Covid-19. UK International Soccer Camps has put in place preventative measures to reduce the spread of COVID-19; however, UK International Soccer Camps cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending a UK International Soccer Camps program could increase your risk and your child(ren)’s risk of contracting COVID-19. By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending a UK International Soccer Camps program and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at the UK International Soccer Camps program may result from the actions, omissions, or negligence of myself and others, including, but not limited to, UK International Soccer Camps employees, volunteers, and program participants and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at the UK International Soccer Camps program or participation in UK International Soccer Camps programming. On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless UK International Soccer Camps, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. Additionally, I acknowledge and agree that photographs and videos may be taken by UK International Soccer Staff for marketing and promotional use only.  I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of UK International Soccer Camps, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any UK International Soccer Camps program.

COMMUNICATION

I, the undersigned, agree to receive relevant emails and text messages, in relation to current and upcoming soccer programs that we have, or will be, registered for.

REFUND POLICY

I, the undersigned parent or guardian of the enrolled named child(ren), understand that should I need to cancel my participation, that the refund will be processed minus a $25 administration fee. However, if I need to cancel within 7 days of the start date, I will only be eligible for a credit on my account and not a cash refund.
